Consent Form

Each interviewee will be asked to sign a Consent Form. When you sign the consent form, you are giving permission to the Indiana Historical Society, WFYI and Storytelling Arts of Indiana to:

• Audio record your interview and take your photograph
• Keep copies of your interview and photograph
• Upload your photograph and edited story to www.lifestoriesproject.net
• Transcribe your interview
• Use your interview and photograph for public access, research, exhibits, publiciity, publishing, programming, performance and events for the Indiana Historical Society, WFYI and Storytelling Arts of Indiana.

Indiana Historical Society, WFYI and Storytelling Arts of Indiana cannot use your interview for commercial purposes without the Interviewee’s written permission.
